A statutory assessment is a process where information is collected from the parents, child/ or young person and professionals. This then allows us to gain a clear picture of the education, health and care needs of the child / or young person.
Applications
A request for statutory assessment can be made by a parent, young person or requested by a child's or young person’s education setting (school, early years setting or college) by downloading and completing the request form.
