Cared for child review consultation form for parents 

The survey will take approximately 4 minutes to complete.
For all Parents and Carers with parental responsibility for a child being cared for by Torbay Children's Services. Please complete this form before the next review to express your views and opinions about your child’s placement and plans that are being made, even if you will not be attending the review.
The results will be kept securely but will be shared with your child's Independent Reviewing Officer (IRO) as part of the review process.
