1. Key purpose of job
- To lead the Resettlement Team on a day to day basis, providing structure, advice and guidance to the work undertaken across the Team.
- To improve and increase potential and expected outcomes, including general wellbeing, for people with multiple and complex needs.
- To assist and support our most complex and vulnerable clients who have lost or are at risk of losing their accommodation to help them move into suitable housing and sustain it.
- To assist, advise and support our most complex and vulnerable clients into relevant supportive services where needs have been identified.
- To provide comprehensive advice and support on housing options with the aim of preventing homelessness wherever this is possible.
- To ensure effective and efficient communication across all relevant teams to ensure a coordinated delivery of supportive and/or other interventions that meet the needs of rough sleepers addressing any other relevant community safety concerns. In particular to work closely with the Leonard Stocks Centre, the Housing First Team, Torbay Council’s ASB and Vulnerability Team and Housing Options Team.
- To promote the safeguarding of adults who use the service.
2. Anticipated outcomes of post
- Resettlement Workers have clearly defined caseloads and the team operates both efficiently and effectively within a wider context of partnership working that reduces rough sleeping and assists access to and the sustainment of accommodation.
- Targeted intervention focusing on engaging with rough sleepers and those most excluded due to complex needs in order to prevent, where possible, homelessness.
- Reduce the demand on Housing Options and associated services through effective management of those who are rough sleeping and/or have complex needs.
- Improved safety, wellbeing, access to / maintenance of accommodation and sustained engagement with appropriate services. Along with reduced social isolation, vulnerability and the risk of harm.
- Maintenance of accurate and up to date information detailing Torbay’s rough sleeping population and bespoke plans of working with them to enable the best response to any identified needs.
- To have more effective partnership working through the implementation of effective working arrangements and information sharing across all relevant teams.
3. List key duties and accountabilities of the post
- To provide day to day advice, support and guidance to the team of Resettlement Workers, allocating incoming referrals across the members of the Team and being the first point of contact for any and all new matters arising.
- To ensure that all record keeping across the Team is maintained to the required accurate standards and that data is produced on a regular basis for the purpose of performance monitoring and the completion of required returns to the government.
- To ensure that the Team adhere to all safe working practices and that staff welfare is maintained as a priority keeping absence to a minimum.
- To produce regular case studies and content for social media purposes to demonstrate the work successfully undertaken by the Team.
- To ensure the implementation of a fair and effective duty rota scheme involving all relevant members of staff and that the appropriate levels of required staffing cover are maintained on a day to day basis.
- To raise any conduct, performance or sickness matters with Line Manager and support them with any further subsequent action(s) where necessary.
- To work with the Leonard Stocks Centre, the Housing First Team, the various parts of Torbay Council’s ASB and Vulnerability Team and Housing Options team to identify rough sleepers and those who are at risk of rough sleeping, share relevant information and coordinate comprehensive interventions.
- To manage a case load and assist other officers with advice, related to complaints/cases received by Housing Options and the wider Community Safety Department that involve elements of vulnerability and homelessness.
- To work with people from the street to accommodation, conducting outreach work on a regular basis.
- To provide direct assistance to clients to attend important appointments commensurate with the aims of the post.
- Work with clients seeking support, by developing a holistic and pastoral approach to care planning and risk management whilst working in collaboration with other services to assess their needs. To identify and work towards each client’s individual goals ensuring an integrated approach towards support and risk management.
- To offer support to individuals with complex needs that are at risk of or have lost their accommodation.
- Assist with the arrangement and delivery of the Severe Weather Protocol (SWEP).
- To provide appropriate advocacy for clients experiencing difficulties.
- Develop positive working relationships with clients in order to facilitate the best possible outcomes and address issues of social isolation and exclusion.
- To complete housing benefit applications with all those accessing temporary accommodation and support those who have been assisted into move-on accommodation.
- To carry out comprehensive assessment of clients and their requirements whilst developing engagement plans for those with complex needs. This duty will include conducting client risk assessments from the start and ensuring such plans are regularly reviewed.
- To use a solution focussed approach in the work undertaken.
- Work in such a way that considers and promotes the safety and wellbeing of the wider community.
- Ensure effective and confidential communication between the local authority and other statutory agencies such as the Police and health care agencies.
- Maintain accurate and timely records and assist with the monitoring of outcomes across the team.
- Conducting all external relations in such a way as to enhance the Alliance’s good standing and reputation within the wider community.
- Proactively work with landlords, provide advice re anti-social behaviour, tenancies, obligations etc to enable and facilitate sustainable tenancies.
- To ensure appropriate safeguarding takes place at all times in line with established policy and procedure times in relation to adults and children identified in the course of duties.
- To be flexible and carry out such other appropriate duties as may be required by the head of service commensurate with the grade of this post.

- Team Leader role in order to take care of day to day caseload management, distribution of new incoming cases and supportive responsibilities.
6. Working environment and conditions of the post
- There will be a normal working environment although there is the expectation that the postholder will attend home visits and undertake assessments in properties and/or to engage with individuals who are rough sleeping on the streets on a regular basis.
- Client group are likely to be challenging and have complex needs, with the majority of engagement being outside of the office environment at times lone working.

- The Council operates a Smoke-Free Policy and the post-holder is prohibited from smoking in any of the Council's buildings (including Council owned and Council leased buildings, but excluding designated areas in residential schemes), enclosed spaces within the curtilage of buildings, and Council vehicles. Staff will not be released for a break that is specifically for smoking.
- The post-holder is expected to familiarise themselves with and adhere to all relevant Council Policies and Procedures.
- The post-holder must comply with the Council’s Health and Safety requirements as outlined in the H&S policy appropriate to the role.
- This post is based at the Town Hall but the post holder may be required to move their base to any other location within the Council at a future date.
- As this post meets the requirements in respect of exempted questions under the Rehabilitation of Offenders Act 1974, all applicants who are offered employment will be subject to a criminal record check (Disclosure) from the Criminal Records Bureau before the appointment is confirmed.
